Crown rot over winter is the biggest single loss
More caraway plants die between autumn and spring than to every pest combined, and the cause is nearly always water rather than cold. The rosette sits flat on the soil surface with its growing point at ground level, so any water that pools there sits directly against the crown for hours. Repeated over a wet winter, that softens the collar, fungi move in, and the plant simply lifts out of the ground in spring with no roots holding it.
You can see it coming. Plants in the low corner of a bed, or in ground that puddles for more than an hour after heavy rain, are the ones you will lose. Plants on a slight ridge, or in ground with grit worked through it, come through unharmed in the same winter and the same weather. This is a drainage problem with a drainage answer, and no amount of row cover or cloches will fix it.
The second contributor is a soft, overfed rosette. Nitrogen applied in the first season builds thin-walled leafy growth and a loose crown that holds water in its center. A lean first year gives a tight, firm crown that sheds water. If you lost plants last winter and the ground drains well, look at what you fed them.
Do not tidy the dead outer leaves off the rosette in autumn. That collapsed ring of foliage sheds water away from the growing point and insulates it. Clear it in early spring instead, once new growth is visible in the center.
Pests, and which ones to ignore
Caraway is in the same family as carrot, parsley and dill, and it shares their pests. Carrot fly is the one that matters if you intend to eat the first-year root: the adult lays at soil level, the larvae tunnel into the taproot, and you find rusty brown galleries when you lift it. The root is ruined for eating, though the plant will often still flower and seed the following year. A two-foot (60 cm) barrier of fine mesh around the bed, or an insect-proof cover over the row through the first summer, is the practical defense.
Aphids cluster on the flowering stems and undersides of the umbels in the second year, sometimes heavily enough to distort a head. They rarely cost you the crop. A strong jet of water knocks them off, and by the time the umbels open there are usually enough hoverflies and ladybirds working the flowers to keep them in check. Insecticides at flowering are a poor trade, because the open umbels are among the best pollinator plants in the garden at that moment.
The striped green and black caterpillars with orange markings that appear on the foliage are swallowtail butterfly larvae, and they are worth leaving alone. They eat a modest amount of feathery leaf that the plant does not miss, and on a second-year plant that is heading for seed anyway the loss is negligible. Move them to another umbellifer if you must, but do not spray them.

The failures that are not diseases
A great deal of what gets reported as a caraway problem is the plant behaving exactly as a biennial should. It made only leaves this year and no seed: that is the first year, and it is correct. It flowered and then died: that is the second year completing its life, and it is also correct. Neither is a fault to be fixed, and the only remedy is to sow every year so that you always have both a first-year and a second-year row standing.
A related one is a second-year plant that stays as a rosette and never flowers. That is usually a winter that was too mild to give the plant the cold spell it needs to switch to flowering. The plant is not lost. Leave it standing and it will flower the following spring instead, effectively as a three-year plant. Digging it up in frustration is the only way to actually lose it.
Then there is the row that never came up. Caraway takes ten to twenty-one days to germinate and can take longer in cold soil, and old seed germinates thinly or not at all. Before concluding the row failed, check the date you sowed it and how old the packet was. Reworking a bed on day twelve destroys a crop that was simply running to its own timetable.
Losing the seed crop, and how to stop repeating it
The most expensive failure in caraway is not a pest at all. It is arriving at the row in the second summer to find bare umbel spokes and the whole crop lying in the soil beneath. The seed ripens unevenly and separates as soon as it is dry, so a head left until it looks uniformly ready has already shed the seed that ripened first, and a windy afternoon or a downpour will finish the job in an hour.
The fix is entirely about timing and it has to be planned rather than noticed. Cut whole stems when about a third of the seed on the head has turned brown and finish the ripening in paper bags under cover. That single change turns an unpredictable harvest into a reliable one, and it costs nothing but being present for a two-week window.
Write down, in the second season, the date the first seed on the row turned brown and the date you actually cut. If those two dates are more than a week apart, that gap is your loss, and next time it can be closed. Note also which plants shattered first, because volunteer seedlings will come up there next spring and they are a free head start on the following cycle.
