What actually attacks it
Water spinach is a vigorous plant and rarely dies of pests, but the soft growing tips are exactly what sap-feeders want. Because you eat those tips, you cannot let an infestation run and deal with it later. Check the underside of the top three leaves on a few shoots every time you water; that is where everything starts.
Aphids are the constant. They cluster on the tips and the leaf undersides, cause curled distorted new leaves, and leave sticky honeydew that goes black with sooty mold. A strong jet of water knocks them off, and because the plant regrows so fast you can also simply cut the infested tips off and take them out of the garden. That is often the quickest fix.
Leaf miners tunnel pale winding trails inside the leaf; squash the larva at the end of the trail between finger and thumb, and pick off badly mined leaves. Flea beetles shot-hole the leaves in dry spells, which is cosmetic and usually stops once the plant is growing hard. Slugs and snails go for seedlings at ground level in wet culture, especially around a standing tray, so check the rim of the tub after dark.

You are harvesting from this plant every week or two. Prefer physical removal to sprays, and if you do spray anything, respect its interval before harvest and remember that the tips you eat are exactly the parts you treated.
Yellow leaves usually mean water or cold, not disease
The most common complaint is yellowing, and it is nearly always cultural. Older leaves going uniformly pale yellow from the bottom of the plant up, on a crop you have been cutting hard, is nitrogen running out. Feed it and the color returns to the new growth within a week, though the old leaves stay yellow.
New leaves coming out pale with the veins still green is iron, and in constantly wet soil that usually means the mix has gone airless and compacted or the standing water has turned stagnant and sour. Flush the tray with fresh water, loosen the surface, and stop feeding until the color improves.
A whole plant going dull yellow-green and simply stopping, with no pests and no obvious cause, is temperature. Below about 60F (15C) this crop stalls, and nights near 50F (10C) damage the tips. Nothing you feed or water will fix a cold plant; either warm it, move the container, or accept that the season is ending.
Rot, stagnation and the one real disease risk
This is a plant that likes standing water, so it is not going to drown, but stagnant water is different from wet soil. If the tray water smells sour, goes cloudy, or grows a thick algal film, the roots are sitting in an oxygen-starved soup. Stems then blacken at the waterline and pull away soft. Empty and refill, and from then on flush the tray weekly rather than only topping it up.
White rust shows as small white or pale blistery pustules on the leaf underside with a yellow patch above. It spreads in humid still conditions and on crowded plants. Pick off affected leaves as soon as you see the first ones, thin the stand so air moves through, and avoid overhead watering late in the day.
Because fragments root so easily, none of this diseased material should go on an open potting mix pile. Bag it, bin it, or bury it deep in a heap you turn and that heats. That habit handles the disease and the containment obligation in one move.
Record the first sighting, not the outbreak
Write down the date you first saw aphids, the date of the first leaf-mine trail, and which end of the bed it started at. Pests arrive on a remarkably consistent schedule in any given garden, and knowing that yours turn up two weeks after the first hot spell means you can be checking before you can see them rather than after.
Note the weather around each problem too. A line saying that the yellowing followed four cool nights, or that the white rust appeared in a humid still week, turns a vague memory into a rule you can act on next season.
