The commonest cause of death is the gardener
It is worth saying plainly, because it is true. More balloon flowers are lost to spring cultivation than to every insect and disease combined. The plant is completely invisible for weeks after the rest of the border has started into growth, so it gets forked through while the beds are being tidied, planted over with annuals by someone who assumed the space was empty, smothered by a mulch dumped over the crown, or lifted and thrown on the compost pile by a gardener who has decided it did not survive the winter.
The prevention is a marker and a note, and it has to be put in place during the season when the plant is standing and obvious. A short cane, a sturdy label, a ring of stones, or all three. Add the position to a sketch of the bed with a measurement from something permanent, because markers get knocked out and labels fade. Tell anyone else who works in the garden what the marker means.
If you find you have cut into the root, do not lift the plant to assess the damage. Firm the soil back, water it in, and wait. A root with a clean cut through part of it often recovers over a season or two, whereas a root that is lifted, inspected and replanted usually does not. The rule with this plant is always the same: less handling, more patience.

Wet feet in the cold months are the real killer
Balloon flower is genuinely hardy and shrugs off cold. What it does not survive is a thick fleshy taproot sitting in cold, saturated, airless soil for months. The plant looks fine going into dormancy, and then simply never appears again, which is why the loss is so often blamed on cold or on a pest. Dig where it was, months later, and you find a soft brown remnant instead of a firm root.
Everything about prevention happens at planting and at soil level. On heavy clay, work a third by volume of coarse grit through the top ten inches (25 cm) of the planting area, and set the crown on a slight rise so surface water runs away from it rather than collecting. In a bed that puddles after heavy rain, use a raised bed rather than trying to improve the existing ground in place. For a plant in a container, the same rule applies twice as strongly: open drainage holes, a gritty mix, and never a saucer left under the pot.
Keep mulch off the crown as well. A generous mulch is good practice around the plant and bad practice over it, because organic matter piled on the resting crown holds moisture exactly where you least want it. Spread mulch in a ring, leaving a couple of inches (5 cm) of clear ground around the center. In autumn, once the top growth has yellowed, cut the stems down to an inch (2 to 3 cm) and clear fallen leaves off the crown rather than leaving a wet mat sitting on it all winter.
Never leave a potted balloon flower standing in a saucer of water through the cold months. A container of cold, saturated compost around a fleshy taproot is the most reliable way to lose one of these plants outright.
Slugs, and the unlucky timing of late emergence
The plant's late start puts its most vulnerable moment squarely in the season when slugs and snails are most active and hungry. The shoots come up soft, fat and juicy through bare ground with no older foliage to hide them, and a clump can be grazed back to stubs over two or three damp nights. Repeated grazing of a young plant, before it has much root behind it, can finish it off entirely.
Check with a torch on damp evenings from the moment the first shoots break the surface until the stems are six inches (15 cm) or so tall and toughening up. Clear leaf litter, boards, pots and loose debris from the ground immediately around the crown, since that is where they shelter through the day. A ring of sharp grit around the crown makes the approach less comfortable and doubles as drainage at the surface.
Beyond that, the pest list is short. Aphids sometimes cluster on soft shoot tips, especially on plants that have been fed generously, and a jet of water directed under the leaves deals with them. Occasionally leaf spot marks the lower leaves in a crowded, damp, still clump: remove the affected leaves, thin the stems to improve airflow, and water at the base rather than over the foliage. Deer and rabbits usually leave the plant alone, though a hungry rabbit will graze new shoots in early spring like anything else.
Flopping, thinning and other disappointments that are not diseases
A clump that falls open from the middle just as it comes into flower is not sick. It has grown too soft and too tall for the strength of the stems, and the causes are rich soil, generous feeding, too much shade or simply a well-grown plant carrying a heavy crop of flowers. The remedies belong to the start of the season, not to the moment of collapse: pinch the tips out at eight to twelve inches (20 to 30 cm), or push a ring or grid support in at about four inches (10 cm) of growth so the stems come up through it.
A clump that flowers thinly is usually short of light, recently disturbed, or too young. Four to six hours of direct sun is the working minimum, and shrubs overhead thicken so gradually that the loss of light goes unnoticed for years. A plant whose root was cut, moved or divided will spend the following season rebuilding rather than flowering, and that is a cost of the disturbance rather than a fault to be treated.
And a display that ends abruptly is nearly always a deadheading failure. Seed capsules left on the stems tell the plant its work is done, and it stops producing buds. Pick the faded flowers off individually every two or three days and the same clump flowers for weeks longer. None of these three problems has anything to do with pests or pathogens, which is why reaching for a spray in any of them is wasted effort.
The notes that prevent the same loss twice
Write down the position of every crown on a sketch of the bed, measured from something permanent, and record the date the first shoots appeared each year. Those two entries between them prevent the commonest way of killing this plant, and after two seasons the emergence date will stop you panicking weeks before there is anything to panic about.
Alongside them, note where water stood after the heaviest rain of the cold months and how long it took to drain, since that is the early warning for root rot a full season before it happens. Record the first date slugs took a shoot, the date you pinched or staked, whether the clump flopped and which way it leaned, and the date the display ended. Over a few seasons those entries turn into a short, specific list of the two or three things your garden actually does to this plant, which is far more useful than any general list of possible problems.
