Scale is the pest that actually matters
If something is wrong with an established bitter orange, look for scale first. These small immobile insects fix themselves to stems, leaf undersides and fruit and feed on sap under a waxy shield, so they look like limpets or raised brown spots rather than anything alive. Run a thumbnail across a suspect bump: a scale lifts off and leaves a damp mark, a natural blemish does not. Heavy infestations sap the tree steadily, give thin pale growth and poor fruit, and their honeydew sets off the second problem.
Scale is easiest to beat when numbers are low and the crawlers, the mobile juvenile stage, are moving. Crawlers appear in flushes in warm weather, and double-sided tape wrapped round a branch catches them and tells you when they are active. Treat with horticultural oil then, covering leaf undersides and the canopy interior thoroughly, and repeat two or three weeks later. Oil smothers, so anything you miss survives, which makes coverage matter more than dose.
Ants are the reason scale keeps coming back. They farm scale for the honeydew and actively defend it from the predators that would otherwise keep it in check, so a tree with an ant highway up the trunk will never clear on its own. Break the route: keep the skirt of the canopy lifted clear of the ground, keep mulch pulled back from the trunk, remove any weed or branch bridging to the tree, and put a sticky band on the trunk if the traffic is heavy.

Sooty mold is the symptom, not the disease
A black soot-like film over leaves and fruit alarms people far more than it should. Sooty mold is a harmless fungus that lives on the sugary honeydew excreted by sap-feeding insects. It does not penetrate the leaf and does not infect the tree. Its only real harm is shading the leaf surface, which reduces photosynthesis if the coating is thick, and making the fruit look filthy. Spraying a fungicide at it achieves nothing at all, because the fungus will simply regrow on the honeydew that is still being produced.
Deal with the insect and the mold stops being replenished. It then flakes away over a few weeks as the leaves grow and rain washes them, and you can speed that up on fruit and on the lower canopy with a soft cloth and a weak solution of dish soap in warm water. On a bitter orange grown partly for the appearance of its winter fruit, that hour of washing before the fruit colors is well spent.
Use the mold as a diagnostic map. It grows exactly where honeydew falls, which is directly below the feeding insects, so the heaviest black coating points you upward to the branch you need to inspect. This is often more reliable than searching for scale directly, since the insects themselves blend into the bark.
Leaf miner, thorn wounds and rootstock breakout
Citrus leaf miner produces silvery winding tunnels through the soft new leaves of the spring and summer flush, often curling and distorting the leaf badly. It looks far worse than it is. On an established tree the damage is cosmetic, the tree carries plenty of unaffected older leaves, and the miner is largely inaccessible to sprays because it feeds inside the leaf. The practical response is to avoid provoking the flush it depends on: do not apply heavy nitrogen in mid to late summer, and do not prune hard in the growing season. On a young tree with little foliage to spare, remove and destroy the worst affected leaves.
Thorn wounds are a genuine problem specific to this species and they run in both directions. Thorns puncture neighboring fruit as it swells, and the puncture becomes a rot entry point, which is why fruit on a congested branch spoils while fruit on an open one does not. Thin the canopy so fruit is not pressed against thorns. Thorns also injure you, and a citrus thorn wound is prone to going septic because the thorn carries surface debris deep into the tissue. Clean any puncture properly rather than ignoring it.
The third specific hazard is not a pest at all. Because bitter orange is the standard rootstock under so many citrus, a rank thorny shoot from below the graft union on any citrus in the garden is very likely to be this species asserting itself. It will outgrow the scion, take the tree's resources and eventually replace it, and the fruit it eventually carries will be sour and useless in that context. Removing it flush at its point of origin, the day you notice it, is the whole of the treatment.
Reach for oil and soap before anything stronger. Broad-spectrum insecticides kill the parasitic wasps and predatory beetles that hold scale and mealybug down between your treatments, and the rebound infestation after a broad spray is routinely worse than the one you sprayed.
Yellow leaves, dropped fruit and split skins
Most yellowing on citrus is a root problem wearing a leaf costume. Yellowing that starts low in the canopy and works upward, on a tree in heavy or poorly drained soil, is waterlogging, and no fertilizer fixes it. Uniform pale yellow with the oldest leaves worst is nitrogen hunger. Yellow between green veins on older leaves is magnesium; the same pattern on the newest tip leaves is iron, usually locked up by wet or alkaline soil rather than absent. Diagnose by pattern and leaf age before feeding.
Fruit drop after flowering is normal and heavy. A bitter orange sets far more fruit than it can support and sheds most of it in two waves, once after petal fall and again at marble size. Do not intervene. Drop that continues beyond that stage, or that takes essentially the whole crop, points to the tree drying out during flowering or set, to a late frost catching the open flowers, or to a wind that stripped the blossom before it was pollinated.
Split skins appear as the fruit colors in autumn and are almost always a watering pattern rather than a disease. A long dry spell followed by heavy rain or a heavy soak causes the flesh to swell faster than the thick rind can stretch, and the rind gives way at the blossom end. Even watering through autumn and a good mulch layer prevent it. Split fruit will not heal, so take it off before it rots on the branch and pulls fungus into the canopy.
Keeping a pest record worth the effort
Note the date you first see scale crawlers each year, the branches worst affected, and the date and product of each oil application. Scale flushes recur at roughly the same point in the season, so a single year of records lets you treat at the right moment next year instead of after the shields have hardened, and hardened scale is far harder to kill.
Record ant activity, the date you banded the trunk, and where the ants were bridging from. Add the date of any fruit splitting alongside your watering notes, because the two lines read together will show you exactly which dry spell caused it, and that is a fault you can prevent rather than treat.
