The plant is the main hazard, not the pests
Lily of the valley contains cardiac glycosides throughout: leaves, flower stems, flowers, rhizomes, roots and berries. These compounds act directly on the heart, and the quantity needed to cause symptoms is small. Nausea, vomiting, abdominal pain, slow or irregular heartbeat, headache and blurred vision are the usual picture. It is treatable and most cases recover, but it is a genuine medical emergency rather than a stomach upset, and children and dogs are the usual patients.
Two routes account for most incidents. The first is the berries. Any flower that has been pollinated ripens through late summer into a berry the size of a small pea, turning from green to a bright coral red that stands out against the foliage, held at knee height or lower in the middle of a soft-looking patch of green. It is hard to design a more attractive object for a small child. The second is the vase water: cut stems leach the same compounds into whatever they are standing in, and a jug of clear water on a low table reads as harmless to everybody.
If a berry, a leaf or the vase water has been swallowed, treat it as urgent. Ring a poisons information service or a doctor immediately, or a vet for an animal, and take a piece of the plant with you so there is no argument about identification. Do not wait to see whether symptoms appear, because the cardiac effects can develop over several hours.
Practical measures that actually reduce the risk: pick the berries off before they color up if children or dogs use the garden, tip vase water down an outside drain rather than a kitchen sink, wear gloves when thinning or dividing, and keep the patch out of play areas and off path edges where a hand naturally reaches.
The wild garlic confusion, which is the dangerous one
Lily of the valley leaves look enough like wild garlic leaves that people have been seriously poisoned foraging. Both are broad, smooth, pointed, strongly veined lengthwise, mid-green, and both come up in shade in spring, often on the same woodland edge and sometimes within a few yards of each other. Cases occur most years, and they occur among experienced foragers as well as beginners, usually because a handful was grabbed quickly rather than looked at.
There are reliable differences, and they take seconds to check. Wild garlic smells of garlic, unmistakably, when a leaf is torn or crushed. Lily of the valley smells of nothing much at all. That single test settles it, provided you test every handful rather than the first leaf you picked, because the two grow intermingled. Beyond the smell, wild garlic leaves each rise on their own individual stalk direct from the ground, while lily of the valley leaves come up in pairs wrapped around each other in a common sheath at the base. Wild garlic leaves are softer and more matt; lily of the valley leaves are thicker, glossier and stiffer.
If you both grow lily of the valley and forage, be careful about the geography. Do not plant it near a wild garlic patch, do not let it colonise the edge of a wood you pick from, and do not pick wild garlic anywhere downhill or downstream of an old garden, where lily of the valley very commonly persists long after everything else has gone. Never pick either plant in the dark or in a hurry.

Pests: very few, and mostly cosmetic
The plant's toxicity keeps most things off it, and rabbits and deer usually leave it entirely alone, which is one of the reasons it is so useful in difficult places. What does attack it is limited and rarely serious.
Slugs and snails graze the emerging shoots in early spring, leaving ragged holes and chewed leaf tips in the pairs that come up first. On an established mat this is cosmetic and not worth treating. On a newly planted patch it can genuinely set the plants back, because those first leaves are all the plant has, so clear the old debris off the ground in late winter to remove overwintering shelter and go out on the first few mild damp evenings with a torch.
Vine weevil is the one to take seriously, and only in containers. The adults notch the leaf margins in a neat scalloped pattern through summer, which is the warning sign; the grubs eat the rhizomes and fine roots in the compost over autumn and winter, and a pot can collapse completely over a single winter with no warning above ground. If you see notched leaves on a potted plant, knock it out in autumn and check the compost, and repot into fresh compost if you find the white grubs.
Aphids occasionally appear on the flower stems in a warm spring and can distort the spike. They are easily rubbed off by hand and rarely warrant anything more.
Diseases: two that matter, both about air and water
Gray mold is the usual disease, and it turns up in a wet spring or on a congested patch where nothing dries out. It shows first as soft brown patches on the flowers and on the leaf tips, then as a fuzzy gray coating in humid weather, and it can rot flower spikes before they open. It is a symptom of stagnant air rather than of infection arriving from elsewhere, so the treatment is thinning: pull out the affected spikes and leaves, take them off the bed rather than composting them, and if the patch is dense, lift and thin it in the following dormant season.
Leaf blotch is the other, showing as pale brown or straw-colored patches with darker margins spreading in from the leaf tips, usually from mid-season onward and usually in damp shade. It looks alarming and is largely cosmetic, since the plant is winding down anyway by the time it becomes obvious. Cut off and remove the worst leaves and clear all the dead foliage off the ground at the end of the season, because that debris carries the spores into next year.
Rhizome rot in waterlogged ground is the only thing that reliably kills the plant, and there is no treatment. A patch that simply fails to appear one spring, in soil that stood wet through the winter, has rotted. The answer is drainage and replanting somewhere raised, not fungicide.
Never treat any of these with something you would not want on a plant you handle bare-handed later. Given how much hand contact this plant involves at thinning time, and how little the diseases actually cost you, cultural control is the right answer here almost every time.
What to write down so problems become predictable
Most trouble with this plant is slow and repeats on a cycle, which makes a few dated notes unusually valuable. Record the date you first see slug damage on the emerging shoots, because next year that tells you when to clear the debris and start looking. Record the season and the position of any gray mold, since it recurs in the same dense corner every wet year and is really a note telling you where to thin.
For containers, record the date you last repotted and whether you found vine weevil grubs. That single line turns an annual autumn check from a chore you forget into a decision you make on evidence. And note the week the berries turn red, because if there are children or animals in the garden that is the week the picking-off job needs doing, and it moves by a fortnight or more between a cold year and a warm one.
