The commonest complaint is not a problem at all
Most reports of sick grape hyacinth are the autumn foliage doing what it always does. The leaves come up months before the flowers, sit through the whole winter, get flattened by frost and rain, and look yellow-tipped and thoroughly ragged by the time the spikes appear. Nothing is wrong, no treatment is needed, and cutting the leaves to tidy them is actively harmful because it starves the bulb.
Distinguishing normal winter wear from real trouble is straightforward. Winter-damaged foliage is dry, papery and firmly attached at the base. Diseased foliage is soft and slimy where it meets the bulb, comes away with a light pull, and usually smells sour. Only the second kind needs acting on.
Summer rot is what actually kills established clumps
Basal rot during dormancy is the one thing that reliably destroys a colony. It comes from soil that stays wet through summer, whether from heavy clay, a downspout, a sprinkler zone, or summer bedding planted over the top and watered all season. The bulbs go soft at the base, the colony thins out over two or three years, and the gaps do not fill in.
The fix is drainage and dryness, not a spray. Stop irrigating over dormant colonies, thin out heavy summer mulch, and if the site itself is the problem, lift during dormancy and replant into a raised or free-draining position. Discard any bulb that is soft at the base rather than replanting it and hoping.
Blue or green mold on lifted bulbs is a storage fault rather than a field disease. It comes from washing bulbs, bagging them in plastic, or holding them too long. Brush the soil off dry, store in open trays, and replant within a few weeks.
Working out what you are looking at
Grape hyacinth is not much troubled by insects. Slugs graze the flower spikes in a wet spring, and deer and rabbits generally leave the whole plant alone. Most of the remaining complaints are cultural, and the table below covers nearly everything people actually see.

The real hazard is the plant itself
The problem most gardeners eventually have with grape hyacinth is not that it struggles but that it wins. It multiplies underground into offsets that thicken the clump, and it seeds freely into lawns, gravel, path cracks and neighboring beds. A planting made from a single bag of bulbs can occupy several times its original area within five years, and it swallows small, slow neighbors under its dense winter foliage.
Removal is genuinely difficult. The bulbs sit at varying depths, the offsets are small enough to pass unnoticed through a fork or a sieve, and any fragment left behind regrows. One thorough dig during dormancy typically reduces a colony by most of its volume and then it comes back thinly the following spring. Getting rid of it properly means digging once and then pulling or hoeing every regrowing tuft on sight for two or three complete seasons.
Because of that, prevention is worth far more than cure. Deadheading before the capsules ripen stops the seed, which is the mechanism that moves the plant into new ground. It will not shrink an existing clump, but it keeps the problem contained to one place instead of scattered across the garden.
Never compost deadheaded spikes, dug bulbs, or offsets. Seed finishes ripening in cut capsules and home heaps do not run hot enough to kill either seed or bulblets. Mulching a bed with that compost seeds grape hyacinth straight into it.
What to record about losses and spread
For rot, note the date you noticed the thinning and the exact spot. Basal rot is a site problem rather than a plant problem, and the same patch of ground will do it again to the next thing you plant there. A note saying which corner stays wet is more useful than any record about the bulbs themselves.
For spread, keep an annual count of how far the furthest seedling was from the parent patch and photograph the colony each spring from the same fixed point. Two or three years of those photographs make it obvious whether your deadheading is keeping pace, which is not something you can judge from memory when the change is a few inches a year.
