Crown rot is the one that kills
Rot at the center is the single most common cause of death, and it comes from water sitting stale in the cup, from cold water sitting in a cold plant, or from a base buried too deep in wet mix. It is quiet. The plant looks entirely normal from a distance while the tissue at the base of the inner leaves softens, then one day a central leaf lifts out at a touch with a sour smell and a wet brown base.
Once the growing point has gone soft there is no cure. What you can do is check the pups: they are usually still sound, and separating one and potting it fresh saves the line. Prevention is the whole game here, and it is simple. Flush the cup regularly, keep the mix on the dry side, never bury the base, and keep the cup shallow when the room is cold.
An early warning is a cup that smells sour when you flush it, or water that comes out cloudy and dark. Flush twice, then check it again in a week.
Scale and mealybug hide where you cannot see
The overlapping leaf bases make a perfect shelter, and both scale and mealybug settle down in the axils where an inspection from above shows nothing. Scale shows as small hard brown or gray discs stuck to the leaf, usually along the underside and near the base. Mealybug shows as white cottony fluff in the joints. Both feed on sap, and the first thing most people notice is sticky honeydew on the leaves or on the surface below the plant, sometimes with black sooty mold growing on it.
Treat by hand first. Wipe the insects off with a cloth or a cotton bud dipped in dilute rubbing alcohol, working right down into the axils, then rinse the whole plant through and flush the cup. Repeat weekly for three or four weeks, because the crawler stage hatches after the adults are removed. Systemic sprays are a poor fit for a plant that holds water in its center, so keep any treatment off the cup and check the label before using anything on a bromeliad.

Damage on the leaves: sun, cold and water
Three different kinds of leaf damage get confused with each other constantly, and they call for opposite responses. Scorch is bleached, dry, straw-colored patches with a papery feel on the surfaces facing the light, caused by too much direct sun, usually after an abrupt move. Cold damage is darkened, water-soaked patches that go brown and soft, appearing after a spell below 50F (10C). Hard water leaves pale chalky spots and a white ring inside the cup, which is cosmetic but permanent on that leaf.
None of this damage heals. The affected leaves stay marked for as long as they are on the plant, and the recovery you are looking for is clean new growth from the center. Judge whether a change worked by the new leaves, not by hoping the old ones improve.

A green plant is not sick
The most common complaint about this plant is not a disease at all. A Neoregelia that has lost its color and grown long soft green leaves is telling you it is not getting enough light. Color is a light response, and no amount of feeding, misting or repotting will produce it in a dim corner. Heavy feeding actively makes it worse by pushing soft green growth.
Move it into bright light with some gentle direct sun, do it gradually over two weeks so it does not scorch, and then wait. The leaves already grown will not change. The color appears on new leaves over the following two to three months. Cut feeding right back to a very dilute quarter-strength solution during the warm months only, and none at all in winter.
Keep a fault log, it pays for itself
Write down the date any symptom first appeared, what the conditions were at the time, what you did about it and whether it worked. For a plant whose responses take months to show, this is the only way to learn anything reliable. A line saying that scale appeared in late winter in the dry heated room, was wiped weekly for four weeks and did not return, is worth more next winter than any general advice.
Record the position of each plant too, if you have more than one. When one plant colors well and another next to it does not, the log tells you what was actually different between them: distance from the window, how often the cup was flushed, when it was last repotted. That comparison is the fastest way to work out what your particular room needs.
