Scale is the pest that matters most
Scale insects are the commonest serious problem on an orange tree and the easiest to overlook, because they do not look like insects. They appear as small brown, gray or white bumps, roughly an eighth of an inch (3 mm), sitting motionless along stems, in leaf axils, on the undersides of leaves against the midrib, and often on the fruit itself. Most people take them for a bark blemish for a year before recognizing them.
They feed on sap, and heavy infestations weaken the tree, thin the canopy, cause leaf yellowing and drop, and reduce fruit size. The infestation builds slowly and then accelerates, so a tree that looked lightly marked last summer can be seriously covered the next. Check the undersides of leaves and the junctions of twigs at every watering in the growing season; that habit alone catches it early enough to matter.
Treat by wiping small colonies off with a cloth or an alcohol-dipped cotton bud, and by spraying horticultural oil for anything larger, covering leaf undersides and stem junctions thoroughly. Repeat after two weeks to catch the mobile crawler stage that hatched after the first spray. Spray in the evening, never in direct sun and never above about 85F (29C), or you will scorch the foliage. Deal with the ants at the same time: they farm scale for the honeydew, defend it against predators, and will restock the tree within weeks if left alone.

Sooty mold is a symptom, not a disease
Sooty mold is the black sticky film that appears on the upper surfaces of leaves, on fruit, and on whatever is standing beneath the tree. It is not attacking the plant at all. It is a fungus growing on honeydew, the sugary waste excreted by scale, aphids and mealybugs feeding above, so its presence is a map: wherever there is mold there is a sap feeder directly above it.
The harm it does is by blocking light. A leaf coated in a black film photosynthesises poorly, and a tree trying to build sugar into a long-hanging crop cannot afford many of them. On fruit it is cosmetic and washes off, though it makes the crop unattractive and the fruit sticky to handle.
There is no point spraying a fungicide at it. Clear the insects causing the honeydew and the mold stops being replenished; existing deposits weather off over a few months or can be washed off with a hose and a soft cloth, working from the top of the tree down. If the mold keeps returning after treatment, you have missed a colony higher in the canopy, or the ants are restocking it.
Leaf miner disfigures but rarely damages
Citrus leaf miner larvae tunnel inside the leaf, leaving silvery, winding, snail-track trails and causing the leaf to curl, twist and look badly deformed. It attacks only soft new growth, so you see it on flushes rather than on mature foliage, and it is worst on late summer and autumn flushes. The damage is alarming out of proportion to its importance.
On an established tree it is essentially cosmetic. The mined leaves still work, and stripping them off is actively harmful because you remove functional leaf area to fix an appearance problem. Leave them on. It matters more on a very young tree with few leaves to spare, where repeated heavy attacks can slow growth.
The useful controls are indirect. Do not feed nitrogen late in the season, because you are otherwise producing exactly the soft flush the moth is looking for at exactly the time it is most abundant. Prune when the tree is not flushing, for the same reason. Sprays are largely ineffective, since the larva is protected inside the leaf, and killing the parasitic wasps that keep the miner in check tends to make the following year worse.
Sprays applied against leaf miner are one of the most common own goals in citrus growing. They rarely reach the larva, they kill the wasps and predatory insects that suppress both leaf miner and scale, and the usual result is a serious scale outbreak in the following season.
Yellow leaves, dropped fruit and split skins
Most non-pest trouble on an orange is a water or root problem wearing a different costume. Even pale yellowing across the whole tree with pale new growth is nitrogen hunger. Yellowing between green veins is a trace element problem, usually iron, and often means locked-up nutrients in alkaline soil rather than an absence of them. Yellowing of older lower leaves with wet soil is drowned roots, and feeding that tree makes it worse.
Sudden drop of small fruit a few weeks after flowering is normal thinning and needs no response. Drop of larger, part-grown fruit points at a water swing, a hot dry wind, or a heavy scale infestation. Fruit that splits lengthwise near the blossom end in late season is almost always the result of heavy watering or rain after a dry spell: the flesh swells faster than the peel can stretch. Split fruit cannot be saved, but even watering through the final swell prevents the next ones.
One problem is genuinely fatal, and it is why the graft union matters. If soil or mulch has been allowed to bury the union, the fruiting variety roots into the ground, loses the rootstock's disease resistance, and becomes vulnerable to collar and root rots. A tree in slow unexplained decline should always prompt you to scrape back the soil at the base and check where that union actually sits.
Keeping a pest record that earns its keep
Pests arrive on a fairly predictable annual cycle. Note the date you first saw scale, leaf miner or aphid, where on the tree it was, and what you did. In most gardens the same pest appears within a fortnight of the same point each year, so the second year's record lets you inspect and act before there is anything visible to react to.
Record treatments with dates, including the follow-up spray, because the second application two weeks after the first is the one people forget and it is the one that decides whether the treatment worked. Note the position of the affected branch too. Scale that returns to the same limb year after year usually means a colony you never fully cleared rather than a fresh arrival.
