Rule out the non-diseases first
By a wide margin, the most common spiraea complaint is a healthy shrub that does not flower, and it is almost never a pest or a disease. Three causes account for nearly all of it: cutting a spring-flowering type during the dormant season and removing the buds, insufficient sun, and heavy feeding that pushes leaf growth at the expense of flower. Work through those three before considering anything else.
The second most common complaint is browning or scorched leaf edges in summer, which is usually drought stress or wind burn rather than infection. Scorch is worst on the exposed side of the shrub and on the colored-leaf forms, and it appears after a hot dry spell rather than spreading gradually from a point. A disease spreads outward from spots; scorch is uniform along an exposed edge.
The third is dieback of individual stems, which on this plant most often means the stem is simply old. Spiraea stems have a useful life of about three or four years, after which they thin, flower less and eventually die back from the tip. Cutting the oldest stems out at the base every year prevents this from ever becoming visible.

Aphids and the other insects
Aphids are the one insect that reliably turns up on spiraea, clustering on the soft tips of new growth in late spring and early summer. They distort the shoot tips, leave sticky honeydew that later goes black with sooty mold, and attract ants that farm them. On an established shrub they are a cosmetic nuisance rather than a threat, and the population usually collapses on its own once ladybirds, hoverfly larvae and parasitic wasps find it.
The right first response is a strong jet of water on the affected tips, repeated every couple of days for a week. That physically removes most of them and does not harm the predators. Broad-spectrum insecticides applied at the first sight of aphids are counterproductive: they kill the predators as well, and the aphids, which reproduce far faster, come back into a garden with nothing left to eat them.
Watch when you spray anything, too. Spiraea in flower is heavily worked by bees and hoverflies, particularly the flat-headed summer types, which are among the better nectar plants in a late-summer garden. If you must treat, do it outside the flowering period and in the evening.
Occasionally you will find leaf-rolling caterpillars, scale insects on old stems, or the leaf damage caused by beetles chewing the margins. None of these usually justifies intervention on an established shrub. Scale is worth attention if it is heavy, and the practical treatment is to cut out the worst-affected old stems at the base, which you were going to do anyway.
Mildew, leaf spot and wet feet
Powdery mildew shows as a white dusty film on the upper leaf surface, worst in the still air at the center of a congested shrub and worst in a season with warm days and cool nights. It is a stress disease as much as an infection: plants that are crowded, in dry soil, or shaded by neighbors get it, and open, well-watered plants in sun mostly do not. The cure is structural. Thin the crown, remove the oldest stems, cut back whatever is crowding the shrub, mulch, and water at the roots rather than over the leaves.
Leaf spots, small dark or purplish marks that appear in wet seasons, look alarming and rarely matter. The shrub carries them through to leaf fall and comes back clean the next year. Rake up and remove the fallen leaves rather than letting them sit under the plant if it was heavily marked, and improve the airflow the same way you would for mildew.
Root rot from waterlogged ground is the one problem that genuinely kills spiraea. The signs are a gradual whole-plant decline: fewer and smaller leaves, stems dying back from the tips, no response to watering or feeding, and soil that stays wet for days after rain. There is no treatment. Either fix the drainage, raise the bed, or move the shrub in the dormant season to somewhere that drains.
Fireblight-type dieback occasionally affects this family, showing as shoots that suddenly wilt and blacken as if scorched while still attached. If you see that, cut well below the affected part into clean wood, take the prunings away rather than composting them, and clean the blades between cuts.
Before treating anything, look at the whole shrub rather than the worst leaf. Damage confined to one side or to the outer exposed growth points to weather or drought. Damage spreading from the crown outwards, or from the base of stems upwards, points to drainage or to old wood, and both of those are fixed with a spade or a saw rather than a bottle.
Winter damage and how to read it in spring
Most spiraea is thoroughly hardy and needs no winter protection in the ground. What you may see in spring is tip dieback, where the last few inches of the youngest shoots have been killed by cold or by drying winter wind. This is normal and cosmetic. Scratch the bark with a thumbnail: green underneath means living wood, brown means dead, and you cut back to the first green.
Do not panic about a shrub that leafs out late. Spiraea often looks completely dead well after other shrubs have broken bud, particularly after a cold spring, and then flushes all at once. Give it until well after the last frost before deciding anything has died, and check the stems with the thumbnail test rather than the calendar.
Plants in containers are the exception on hardiness. A rootball in a pot freezes right through in a way that ground soil does not, and that can kill a shrub that would be perfectly hardy planted out. Move pots against a wall, group them together, or sink them into the ground for the winter in a cold climate.
What to record when something goes wrong
Write down what you saw, the date, and where on the plant it was: the tips, the shaded side, the crown, or one stem in isolation. Position on the plant is the most diagnostic single detail and the one people forget by next season. Add the weather in the preceding fortnight, since most spiraea troubles trace back to a hot dry spell or a long wet one.
Note anything you did in response and whether it worked. Aphids in particular return to the same shrubs at roughly the same week each year, and two seasons of dated notes let you look for them a week early and knock them off with water before the population builds instead of dealing with sticky leaves in midsummer.
