Wet roots, not pests, are the main enemy
The overwhelming majority of failures with Madagascar periwinkle are root and stem rots brought on by cold or waterlogged soil. The pattern is familiar: a plant put out early, watered generously, sitting in ground that stays damp. Within two or three weeks the lower leaves yellow, the stem darkens near soil level, and the plant either collapses in a day or fades over a fortnight.
The trigger is nearly always a combination rather than a single cause. Soil below about 65F (18C) slows root function while the leaves keep transpiring, so the plant is under stress before any pathogen arrives. Add standing moisture and the soil-borne fungi that cause damping off and stem blight have everything they need. Warm soil and dry-ish conditions largely remove the problem without any spray.
There is no cure once the stem base has gone soft and brown. Pull the plant, and do not put another one straight into the same spot without improving the drainage first, because whatever killed the first will still be there.
Wilting is not proof of thirst. Check the soil with a finger every single time. A wilting plant in wet soil is drowning, and watering it again is the most common way people kill this plant.
Telling the common problems apart
Use the leaves and stem base together. Rot starts low and works up: yellowing at the bottom, a darkened or shrunken stem at soil level, and a plant that pulls out with barely any root. Leaf diseases start as discrete spots on individual leaves and spread outward across the plant rather than up from the ground. Insect damage leaves visible marks, honeydew or webbing, and the plant otherwise grows normally.
Gray mold can take hold in a cool wet spell, especially where plants are crowded or where dead leaves have collected inside the mound. It shows as fuzzy gray growth on collapsed tissue, usually starting on damaged or dying parts. Remove the affected material, open the plants up, and stop overhead watering.
Aphids cluster on soft new tips and cause distorted growth with sticky residue; a jet of water or finger-and-thumb work handles small numbers. Spider mites appear in hot dry conditions, especially on plants under cover, showing as fine pale stippling and webbing on leaf undersides. Slugs damage young plants at ground level early in the season but rarely bother established ones.

Prevention that actually works
None of this needs a spray cabinet. Warm soil, sharp drainage, generous spacing and restrained watering handle nearly everything this plant is prone to, and a bed managed that way normally goes a whole season with no intervention at all.
If a spray is genuinely needed for a leaf disease, treat it as a stopgap while you fix the conditions. Repeated spraying of plants that are crowded in damp ground buys a few weeks and no more.
- Plant late into warm soil Wait until night lows hold above 55-60F (13-16C) and soil at planting depth reads above 65F (18C). This one change removes most of the rot problems people fight all season, and costs only a fortnight of growing time that the plant makes up quickly.
- Fix drainage before you plant Work coarse grit into heavy ground and set plants on a slight mound so the stem base sits above the surrounding soil. In pots use a free-draining mix, check the holes are clear, and never leave a saucer with water standing in it.
- Space for airflow Set plants 10-12 inches (25-30 cm) apart so air moves between them once they mature. Crowded plantings hold moisture on the leaves for hours after rain, which is what leaf spot and gray mold need to establish.
- Water the ground, early in the day Water at the base rather than over the top, and do it in the morning so any splashed foliage dries within the hour. Water only when the top inch or two (3-5 cm) of soil is dry, checking by finger rather than by routine.
- Clear debris out of the mound Pull dead leaves and any fallen flowers out of the center of the plant every few weeks. Decaying tissue trapped inside a dense mound is where gray mold and stem rot most often begin.
- Move the planting each year Do not plant into the same bed or the same pot of old compost season after season. Soil-borne rots build up, and a spot that lost plants last year is very likely to lose them again.
It is poisonous, so handle it accordingly
Every part of this plant is toxic if eaten, by people and by pets, and the sap can irritate skin and eyes. It contains potent alkaloids, which is exactly why it has a long history in medicine, and it is not a plant to experiment with in any home preparation.
In practice this means placing it away from spots where small children play or where dogs and cats chew at plants, washing your hands after pinching or pulling it, and not composting large amounts of it where livestock or poultry might reach the heap. Wear gloves if you are pulling out a bed of it at the end of the season.
Knowing when to stop trying
A plant whose stem base has gone soft and brown will not recover, no matter how it is treated. Pull it, and check the neighbors on either side for the same darkening at soil level, because rot spreads slowly through a crowded row. Replacing one plant in early summer is easy; nursing a dying one for a month means an empty gap in high summer instead.
Keep a short note of which positions lost plants, in what week, and what the weather had been doing. Over a couple of seasons this turns into a map of the corners of your garden that stay too cold or too damp for this plant, and it is far more reliable than judging a spot by eye in spring when everything looks fine.
