Scale insects and the sooty mold that follows
Scale is the defining pest of home citrus, and grapefruit's dense evergreen canopy suits it well. The insects look like small brown, gray or white bumps stuck along stems, on leaf midribs and on the fruit itself. They do not move once settled, which is why people often mistake them for a growth or a scab rather than a living insect.
You usually notice the consequence before the cause. Scale excretes sticky honeydew, which coats the leaves below it, and a black sooty mold grows on that honeydew. Leaves go tacky and then black, and the tree looks filthy. The mold does not attack the tree directly but it blocks light from the leaf surface, and a heavily blackened grapefruit photosynthesises poorly at exactly the time it should be building sugar in a long-hanging crop.
Ants are the reliable early signal. A steady column of ants running up and down the trunk means honeydew above, and it also means the ants are actively protecting the scale from the ladybirds and parasitic wasps that would otherwise control it. Breaking the ant trail is often more effective than anything you spray. A band of horticultural glue or a sticky barrier around the trunk, with any branch that touches a fence or a wall pruned clear so it cannot be used as a bridge, cuts the supply line.
For the scale itself, a thorough horticultural oil spray smothers it, applied to cover the undersides of leaves and the stems where the insects actually sit. Spray in mild weather, never in strong sun or above about 90F (32C), and never on a drought-stressed tree, or you will scorch the leaves. Repeat two or three weeks later to catch the crawlers that hatched after the first pass. On light infestations on reachable branches, wiping the scale off with a cloth works and does no collateral damage at all.

Leaf miner, aphids and the flush they follow
Citrus leaf miner attacks only the young soft flush, tunneling silvery trails inside new leaves and leaving them curled and puckered. On a mature grapefruit it is essentially cosmetic. The tree makes several flushes a year and can carry the damage without any loss of crop, and the parasitic wasps that control it do a good job if you do not spray them out of existence. Removing affected leaves makes things worse, since you are taking away the leaf area the tree still uses.
Aphids cluster on the same soft growth, distorting new leaves and producing the honeydew that feeds sooty mold. On an established tree they are a passing spring problem that predators clear up. A strong jet of water knocks them off effectively and costs nothing. Reach for insecticide only on a young tree that is genuinely being held back, and understand that broad-spectrum sprays remove the predators too, so the next outbreak is usually worse.
Both pests are far heavier on trees that are pushed with nitrogen. If you are getting repeated bad infestations, look at your feeding schedule before you look at your spray cabinet: soft, fast, over-fed growth is the actual invitation.
Note the week you first see leaf miner tunnels, ant columns or aphid clusters each year. After two seasons the dates repeat closely, and you can put a sticky band on the trunk or check the undersides of leaves a fortnight before the trouble arrives rather than a month after.
Split fruit, dropped fruit and marked rind
Grapefruit splitting is a watering problem, not a pest. The rind stops expanding as the fruit matures, so when a dry spell is followed by heavy watering or rain, the flesh swells faster than the rind can accommodate and the fruit cracks, usually at the blossom end. Nothing can be done for a split fruit; take it off so it does not rot on the branch and draw wasps and ants into the crown. The prevention is entirely in your watering: keep the interval even through the second half of the season, and mulch to buffer the soil against sudden change.
Fruit drop alarms people far more than it should. Grapefruit sets vastly more flowers than it can carry and sheds in two waves: a large drop shortly after petal fall, and a second smaller drop several weeks later, which northern-hemisphere growers know as the early summer drop. Both are normal and both are the tree matching its crop to its resources. Do not thin, feed or intervene until the second drop has finished.
Drop outside those two waves is a signal. Fruit falling steadily through summer usually means water stress in one direction or the other, so dig down and check the soil rather than assuming drought. Fruit dropping after a cold snap is frost damage. Fruit dropping with a small hole or a soft blemish should be cut open and inspected before you decide anything.
Marked rind is mostly harmless. Wind rub from a thorn or a twig leaves a corky scar, sunburn leaves a pale leathery patch on the exposed side, and cool-weather blemishes are common. All of these look bad and taste of nothing; the flesh under them is fine. Use marked fruit first, since a broken rind will not store, and cut a puncture open to check whether anything went in through it.
Yellow leaves: read the pattern before you act
Yellowing is the symptom people report most and the one most often treated wrongly. The pattern of the yellowing, and which leaves show it, identifies the cause far more reliably than any general fertilizer will fix it.
Uniform pale green across the whole tree, oldest leaves worst, is nitrogen hunger, and it is common on a grapefruit that has not been fed enough for a heavy year-long crop. Yellow between the veins on older leaves, with a green inverted V shape remaining at the leaf base, is magnesium. Yellow between the veins on the newest leaves, with the veins themselves staying sharply green, is iron, and here comes the trap: iron chlorosis on citrus is very often caused by soil that is too wet or too alkaline for roots to take iron up, not by an absence of iron in the soil. Applying more iron to waterlogged ground changes nothing.
Yellowing with leaf drop, on a tree that also wilts, is a root problem. Dig down six inches (15 cm) at the canopy edge and feel the soil. Wet soil under a wilting tree means the roots are drowning; stop watering entirely and improve drainage, and expect the tree to take a season to recover. Dry soil means the opposite and needs a deep slow soak.
A few yellow leaves dropping from the inside of the canopy in spring, while the outside pushes new growth, is simply the tree renewing itself. Evergreen does not mean the same leaves forever. Keep a running note of which pattern you saw, what you did and what happened over the following six weeks. Citrus responds slowly, so without a written record you will genuinely not remember whether last year's treatment worked, and you will repeat the guess.
