Crisp brown leaf edges: scorch, not disease
This is by far the most common complaint about Japanese maples, and it is almost never an infection. The leaf margins turn dry, papery and brown, often starting on the side of the tree facing the afternoon sun or the prevailing wind. The brown is crisp to the touch, the boundary with the green is fairly sharp, and there are no spots, rings, mold or ooze anywhere.
What has happened is simple water arithmetic. The thin leaves lose water faster in hot, moving air than the shallow roots can pull it from the soil, and the tissue furthest from the veins, the margins and the tips, dries out first and dies. It is a physical injury, not an organism. Fungicides do nothing, and neither does feeding.
So the answer is where the tree is planted, not what you spray on it. Deepen the mulch, water thoroughly and less often to send roots downward, and put up a windbreak or shade on the exposed side if you can. If a tree scorches badly every single year, move it in the dormant season to a spot with afternoon shade and shelter. That is the cure; everything else is management.
The finely dissected, thread-leaved kinds burn far more readily than the broad-lobed ones, because a deeply cut leaf has much more edge relative to its area and every bit of that edge is exposed. If you are growing a finely cut form, treat what would be a moderate position for a broad-leaved tree as too hot for this one.

Wilt disease: the one that is genuinely serious
The pattern that should worry you is not gradual browning across the whole tree but a single branch, or one side of the tree, wilting suddenly while everything else stays green and turgid. Leaves droop, hang on rather than falling, and dry on the twig. This is the signature of a wilt organism living in the water-conducting tissue, and it comes up through the roots from the soil.
Confirm it by cutting through an affected branch and looking at the cut face. Healthy maple wood is uniformly pale. Infected wood shows a dark green, olive or brownish staining, sometimes as a broken ring, sometimes as streaks. That discoloration is the blocked vessels, which is why the branch cannot get water.
There is no cure to apply. What you can do is remove the affected branch, cutting well below the last visible staining into clean wood, and disinfect the saw or pruners between every cut so you do not carry the fungus into healthy tissue. Then keep the tree evenly watered and mulched and stop feeding nitrogen. A tree under no other stress will often wall the infection off and live for years. A tree that is also drought stressed usually will not.
Do not replant another maple in the same spot if a tree dies of it, because the organism persists in the soil. Grasses and conifers are not susceptible and are safe replacements there.
Sterilize pruning tools between cuts whenever you are cutting out anything you suspect is a disease. Wipe the blade with household disinfectant or surgical spirit and let it dry. It takes ten seconds and it is the only thing standing between one infected branch and three.
Aphids, scale and the sticky black film
If the leaves are sticky and a black sooty film builds up on their upper surfaces, look underneath and at the shoot tips. You will find aphids clustered on soft new growth, or the flat, limpet-like bumps of scale insects on the twigs and stems. Both feed on sap and excrete honeydew; the sooty mold simply grows on the honeydew and is a symptom rather than a disease of the tree.
Neither is usually life-threatening on an established maple. Knock aphids off with a jet of water from a hose, repeated over a few days, and leave the ladybirds, hoverfly larvae and small birds to finish the job. Avoid broad-spectrum insecticides, which kill those predators and reliably make the next outbreak worse. Scale on a small tree can be rubbed off by hand in late winter when the twigs are bare and easy to work along.
The reason an outbreak took hold is usually soft growth. Heavy nitrogen feeding produces exactly the sappy shoots aphids prefer. Cutting out feeding altogether is often the whole treatment. Once the insects are gone the sooty mold weathers off over the following season, or you can sponge it off the worst leaves with plain water.
Dieback, bark wounds and frost damage
Japanese maples do not seal wounds quickly, and small dead twigs at the outside of the canopy are normal in any established tree as it shades out its own inner growth. What is not normal is dieback progressing down a branch toward the trunk, with sunken or cracked bark and small colored pustules on the dead wood. Cut that out promptly, taking the cut back into clean, live wood, and do it in dry weather.
Mechanical damage matters more on this tree than on most. The bark is thin, and a string trimmer nick or a mower knock at the base creates a wound that stays open for years and lets decay in. Keep grass away from the trunk with mulch rather than machinery.
Late frost after the buds have opened is a seasonal event rather than a disease. New shoots go limp and brown overnight, and it looks alarming. Leave them alone. The tree carries reserve buds and will push a second flush within a few weeks. Cutting the damaged wood off immediately usually removes live buds you will want later; wait until early summer, when you can see exactly what is dead.
Keeping a problem log
For each problem, write down the date you first noticed it, which part of the tree it was on, which way that part faces, and what the weather had done in the preceding week. Scorch and wilt look similar to a worried owner in the moment but their records are completely different: scorch appears on the sun-facing side after a hot dry spell and affects margins everywhere, while wilt takes out one whole branch regardless of aspect or weather.
Note every cut you make to remove diseased wood, with the date and the branch. If dieback returns to the same branch two years running you are dealing with something in the wood rather than a one-off injury, and that distinction only becomes visible in a written record.
